To Join our club you must fill out the application form below and submit it to us, along with a current full body picture photo of you and your partner. 

Both photos must be submitted for approval. 

 

We want to know you better!  Please include a brief description in the Message Box. Describe yourself, tell us about your hobbies and a little about your personality and feel free to include a little note about your dreams and desires.

All New Applicants Must Submit

 

-A Recent FULL BODY  , FRONTAL PHOTO WITH FACE to be considered.

-IT CAN BE A PICTURE OF BOTH TOGETHER OR SEPERATE 

-Both Parties Are Required 

PICTURES WITH BLOCKED FACES , BLURRY FACES OR NO FACES WILL NOT BE CONSIDERED FOR APPROVAL AND MEMBERSHIP WILL BE DENIED. 

 

No Exceptions - This Is For The Benefit Of All Members.  

*NOTE* If you are not the person(s) in the photo you had submitted for approval you will be denied entry upon arrival to Checkmate.

* All photos are removed and deleted permanently after viewing.

IF APPROVED, YOU WILL RECEIVE AN INVITATION FROM US. OUR INVITATION MUST BE PRESENTED AT THE DOOR IN ORDER TO BE ALLOWED INSIDE,

 

WE DO NOT TAKE WALK-INS. WITHOUT THE INVITATION YOU WILL BE TURNED AWAY.

YOUR MEMBERSHIP BECOMES ACTIVE AFTER THE FIRST TIME YOU ATTEND OUR CLUB.

YOUR MEMBERSHIP WILL BE CANCELED IF YOU DO NOT ATTEND OUR CLUB FOR MORE THAN A YEAR, A MEMBERSHIP RENEWAL FEE WILL BE CHARGED IF YOU WANT TO BECOME A MEMBER AGAIN AFTER THE INACTIVE YEAR.
